FACULTY EVALUATION INFORMATION

1.         Faculty Evaluation Form Ratings

Rating of “4” – Faculty member went beyond the required criteria with no assistance

Rating of “3” – Faculty member met all required criteria

Rating of “2” – Faculty member met some but not all required criteria.

Rating of “1” – Faculty member did not meet the required criteria to the satisfaction of the Department Chair.  

 

2.         To achieve a rating of “4”, faculty member must present evidence-bearing documentation for each specific item to the supervisor conducting the evaluation. 

 

3.         Ratings of “3”, “2” and “1” are self explanatory; therefore, no examples are provided for these ratings.

 

4.         Faculty Evaluation Form rating “4” examples.

 

  • ITEM 1:  Uses a current course design document for each assigned course.
    • Provides documentation of annual course validation such as course instructional plan, unit plans, lesson plans, textbook review, test validation, etc.
    • Provides documentation of contributions made to course content such as unit slides, lab development, videos, supplemental course materials, etc.
    • Faculty member develops or revises a course based on the results of task surveys as directed by the Department Chair. 
    • Provides evidence of incorporating media formats via technology.

 

  • ITEM 2:  Provides a course syllabus to students indicating course objectives, outcomes, text and material lists, course evaluation methods, office hours, and other information as required or appropriate.
    • Provides evidence of course syllabus development, revisions and updates.
    • Provides copies of course syllabus with revisions indicating individual instructor information.  
    • Provides evidence where the listed information is communicated, in writing, to students within the first 5 days of class:  course objectives, text and material lists, evaluation methods and due dates for projects.

 

  • ITEM 3:  Creates a positive environment which promotes student involvement through the use of different teaching/learning strategies relevant to the identified course.  
    • Provides evidence of student participation in hybrid courses.
    • Provides evidence of the use of collaborative projects.
    • Provides survey results of student feedback regarding content that illustrates learning style differences such as screen shots of material presented in non-traditional ways.

 

  • ITEM 4:  Develops, revises or updates instructional material to enhance student learning.
    • Provides evidence of integration of new instructional resources accompanying new editions of textbooks.
    • Provides evidence that the faculty member developed innovative instructional materials and shared them with the department for classroom implementation.  

 

  • ITEM 5:   Incorporates important college or departmental initiatives into courses.
    • Provides evidence of incorporating critical thinking activities a SIGNIFICANT part of courses.
    • Provides evidence where course extra credit was offered for student volunteers in college and/or departmental activities.
    • Provides evidence of the utilization of a variety of technology in the classroom.
    • Provides evidence that the faculty member developed a departmental initiative that was implemented by the department.

 

  • ITEM 6:  Complies with departmental and college policies and procedures, i.e., office hours, assigned faculty workloads. 
    • Provides evidence that the faculty member competently covers classes for other faculty members who are out on leave.   
    • Provides evidence that the faculty member devotes time beyond scheduled class/lab/office hours to provide students additional instruction.

 

  • ITEM 7:  Meets student advisement and/or career related needs.
    • Provides evidence of active student advisement such as a student-signed advisement form.   
    • Provides job hunting skills, resume writing tips, interviewing skills, etc. such as documentation of class participation in Job Placement Office workshops.

 

  • ITEM 8:   Maintains and documents progress towards current professional growth plan.
    • Provides evidence of maintaining a monthly calendar showing professional growth progress.
    • Provides evidence that the faculty member obtains a minimum of 20 hours of professional development annually.  
    • Provides evidence that the faculty member received an advanced degree or certification, attended college classes or certification classes or made significant enhancement to teaching due to the application of professional growth activities.

 

  • ITEM 9:   Contributes to the operation and growth and development of the department.
    • Provides evidence of actively promoting/recruiting for the department.
    • Provides evidence of establishing articulations with Dual Enrollment, College Connections, and/or Special Projects.
    • Provides evidence of establishing affiliation agreements with prospective affiliates within the community.
    • Provides evidence of faculty member participation in community-related activities, representing their respective programs.

 

  • ITEM 10:  Contributes to the operation and growth and development of the college
    • Provides evidence of faculty member participation in recruitment activities within the community, representing the college as a whole.
    • Provides evidence of faculty member participation in community-related activities, representing the college as a whole.
    • Faculty member published an article in a professional journal

 

  • ITEM 11:   Works collaboratively with colleagues
    • Faculty member served as the leader of a PAIT team within the past year.
    • Faculty member served as a member on two PAIT teams within the past year.
    • Faculty member served as a mentor within the past year.

 

  • ITEM 12:  Serves on college committees
    • Faculty member serves on a minimum of three committees annually and provides copies of the minutes showing attendance.
    • Faculty member serves on a minimum of two committees annually and holds an office in one committee and provides copies of the minutes showing attendance.

 

  • ITEM 13:  Serves as a club sponsor and/or supports out of class student college related activities i.e. BPA, Skills USA, Miss/Mr TSTC, etc.
    • Provides evidence that the faculty member has participated in one or more out-of-class college activities.
    • Faculty member has participated in or supported a minimum of 3 fund raising events sponsored by or in conjunction with the student government association, providing evidence of participation and/or donations.
